Description of Services: Provides education to enhance the knowledge and skills for staff working in primary care clinics, long-term care homes and community support agencies, who service older adults with cognitive impairment due to mental health, dementia, addictions or other neurological conditions

These service can be used by:
  • Primary care clinics including Family Health Teams and Community Health Centres
  • Seniors' wellness programs
  • seniors' assisted living programs
  • Community care agencies
  • Assigned adult day programs
  • Family Caregivers

Eligibility - Population(s) Served: Must be a government funded agency such as: Ontario Health atHome-Brampton; Adult Day programs; Homemaking agencies; Supportive Housing; Respite Care programs dealing with dementia and mental health issues
Application: Referrals can be made by anyone * within a government funded community agency * who has identified a learning need related to the mental health of the elderly * or anyone concerned about an older adult who may have a psychological, behavioural or memory problem
