Description of Services: A partnership between Services and Housing In the Province (SHIP), Punjabi Community Health Services, and Peel Senior Link * offers supportive and health services that maximize the ability of high risk seniors to live independently in their homes

Makes in-home visits within North West Brampton

The team is comprised of counsellors, mental health counsellors, case managers and personal support workers. Services include:
  • 24-hour on call system for emergency crisis situation
  • risk management assessment and education
  • personal support services
  • access to supportive housing options
  • chronic disease management through linkages with existing community-based health programs
  • falls prevention training, assessment and management of environmental and home risk factors

Eligibility - Population(s) Served: Older adults aged 55 and over, who have complex physical, emotional and medical concerns
At-risk older adults who would benefit from health education, crisis prevention, chronic disease self-management and intervention for social isolation, covers North West Brampton area
