Description of Services: A supervised outpatient program designed to improve heart health. The program helps individuals recover after experiencing a heart attack, angioplasty (stent), bypass surgery, valve replacement or heart transplant

Serves people who:

  • have chronic heart disease
  • have had heart surgery, or
  • have had a recent heart transplant

Helps prevent the disease from progressing and avoid any repeat heart attacks. Program focuses on education, exercise and positive lifestyle choices

Offers in-person and virtual sessions once a week over the course of 12 weeks

Weekly visits include:

  • interactive heart healthy education
  • aerobic and resistance training
  • flexibility/mobility sessions

Additional services:

  • virtual education session with a registered dietitian
  • virtual cognitive behavioural therapy, offered to eligible patients dealing with anxiety and depression

Fees: Most fees covered by OHIP
Eligibility - Population(s) Served: Adults 18 years and older with cardiac disease or who have high risk of developing cardiovascular disease
