Description of Services: Supportive Housing for people with problematic substance use by providing stable housing and appropriate support services. The program is based on the Housing First model, which provides supportive housing to people without requirements for engagement in addiction treatment or abstinence. The program is open to individuals 16 and older, who have problematic substance use and require stable housing.

Eligibility - Population(s) Served: Homelessness / At Risk Only
Application: Referrals are accepted from anyone, including self-referrals. Referrals are through The Access Point. Please call or fill out the application form online. Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment.
