Description of Services: | Short-term complex medical management program to help patients requiring ventilation to breathe reintegrate into the community from acute care * provide rehabilitation, training, and education | ||

Fees: | Service and standard ward accommodation covered by OHIP * inpatients unable to return to independent living after program and are waiting for placement in a facility providing an appropriate level of care must pay a daily chronic care co-payment | ||
Eligibility - Population(s) Served: | Adults 18 years and older who require mechanical ventilation for all or part of the day * must be medically stable * must be cognitively stable (or have a documented substitute decision-maker) * must be willing to participate in the program * must be able to direct own care * must be able to return to the community with established discharge support | ||
