Description of Services: |
Provides conversation group therapy for people with aphasia, an acquired language disorder caused by stroke, brain injury or brain illness. Follows a life participation approach - enhancing communication skills and quality of life of people with aphasia. Provides support to family members and caregivers and educates the community about aphasia. The goal of the aphasia program is for group members to participate actively in conversation using a variety of communication strategies. A typical session may include:
Sessions are facilitated by Speech-Language Pathologists and Communicative Disorders Assistants with the support of trained volunteers and students Services take into account the client’s perspective and current communication abilities. The programming is designed to reflect the client’s varying needs, wants, and abilities Aphasia is an acquired language disorder, most often caused by stroke, brain injury or brain illness, that affects a person's ability to communicate. Aphasia does not affect intelligence |

Eligibility - Population(s) Served: | Varied types / severity of aphasia due to stroke or brain injury, e.g., Acquired Brain Injury (ABI), Primary Progressive Aphasia (PPA), tumor * individuals must be medically stable and living in the community * willingness to participate in supportive conversation groups * demonstrate appropriate behaviour in a social group setting * No / minimal cognitive difficulties * client care needs (i.e., toileting, feeding, and arrangement of transportation) are met by client or caregiver. | ||
Application: | Referral forms can be found on our website (www.h-pcap.com) and faxed to 365-601-1690. Referrals can be made by Professionals in the community (Discharge Planners, Speech Language Pathologists, Social Workers, Physicians, etc.), Self-referrals (clients can refer themselves to the program), Family or Friends of the person with aphasia. | ||

Halton-Peel Community Aphasia Programs are social communication groups for people living with acquired communication disorders. Our programs involve both small and large group activities and are intended for people who want to improve their communication. As an organization, we educate the community about aphasia and provide training and support to family members, friends, and caregivers.
