Description of Services: Medical care team focused on community health for homeless and under housed residents  * coordinates patient care in order to assess and treat injuries or illness * team includes family doctors, nurses, nurse practitioners, social workers, and community resource workers

Services include:
  • Health Bus: deliver mobile on-the-spot nursing care to homeless and under-housed people in the southeast Toronto neighborhood, as well as other parts of the city
  • Hep C Program: a team of health professionals providing care for people living with Hepatitis C 
  • Take Home Naloxone Program: training and naloxone kits for free to current and former opioid users, their friends, and family * do not need to be a client at Sherbourne or show ID

Fees: Most services covered by OHIP
