Description of Services: Family health team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family doctors, nurses, nurse practitioner, social workers, registered dietitian and pharmacist * travel medicine consultation available (appointment required) * chronic disease management, disease prevention and health promotion * seasonal flu vaccine and childhood and adult immunizations * cancer screening tests
