Program: Assertive Community Treatment Teams (ACTT)

Organization: Services and Housing In the Province (SHIP)
Description of Services: Two ACTT teams serve North Peel and North Mississauga * address the ongoing complex needs of people with a mental illness living in the community * offer community-based treatment, rehabilitation and support

Multi-disciplinary teams comprised of psychiatrists, psychiatric nurses, occupational therapists, social workers, peer support workers, addiction/concurrent disorder counselors, vocational counselors, mental health clinicians

Offer assistance with:
  • daily living
  • employment and education
  • budgeting
  • integrated treatment for co-occurring substance usage
  • disease prevention, supportive counseling and health training

Supports available 24 hours a day, 7 days a week

Eligibility - Population(s) Served: Persons age 16 or older with mental illness having complex needs
Application: Referrals are processed though Central Intake
