Organization: Reconnect Health Services
Description of Services: Case management services are offered to individuals living in the Home and Community Care Support Services Toronto Central Region. This recovery-based service includes assistance with finding and maintaining housing, life skills training, accessing community resources and building support networks. Staff liaises with other supports and work collaboratively with them.

Application: Referrals are made through The Access Point. Referrals can be made by family members, physicians, hospital professionals, mental health and community organizations. Self-referrals are encouraged. To make referral through The Access Point, call or complete the online application form.
