Description of Services: An intensive community-based mental health service provided by a multidisciplinary team for people with a severe and persistent mental illness, functional impairments and complex needs. The program provides psychiatric assessment and treatment, rehabilitative and support services including advocacy, life skills training and accessing housing, financial and legal services. The service is for adults 16 years and over with severe and persistent mental illness. The individual must have high needs which may include: high use of hospital service; significant impairments in activities of daily living; co-occurring disabilities such as substance use and developmental delay; involvement in the criminal justice system; and inability to participate in other community-based services.

Application: Referrals for are through The Access Point. Referrals can be made by physicians, hospital professionals, mental health and community organizations. Self-referrals and referrals by family members are also accepted. Eligibility Criteria: To be eligible for service, an individual must: - be at least 16 years of age - be experiencing severe, persistent mental illness with complex symptoms and issues - have a lot of difficulty functioning in everyday life and may have had several hospitalizations. - be able to be safely supported in the community.
