Description of Services: Referral program that links individuals and families affected by Alzheimer's disease or related dementia to a community of learning, services and support
  • provides access for individuals living with dementia and their caregivers to comprehensive and coordinated services
  • people and families are connected with support from a Dementia Resource Consultant when a First Link Referral is made by health care professionals or community agencies
  • caregiver support groups and counselling sessions
  • increased effective and efficient utilization of community resources
  • one-on-one non-therapeutic dementia resources consulting and group supports for care partners, including family and friends

Eligibility - Population(s) Served: People living with mild cognitive impairment (MCI), Alzheimer Disease or related dementias, and their caregivers
