Description of Services: |
Adult Day Program (known as Clubs of Acclaim Health) * social, recreational, and therapeutic activities for older adults, frail elderly and people with disabilities in a group setting for part of the day * helps to support family caregivers who provide day-to-day care * assistance with eating, walking, toileting and taking medications (no injections) * transportation is not included in program fee * virtual club adult day programming also available Participants may participate in any of the following activities:
